PRE-EXISTING INJURIES & CONDITIONS. If I have any pre-existing injury, physical condition, chronic illness, recent surgery, cardiovascular condition, or any other health concern, I understand that I MUST consult with my licensed physician or qualified healthcare provider BEFORE participating in any physical activity at this event. By agreeing below, I confirm that either (a) I have received medical clearance to participate, or (b) I have no known conditions that would prevent safe participation.
PHYSICIAN CLEARANCE RECOMMENDED. Even without a known condition, I understand it is recommended to consult a healthcare provider before beginning any new exercise program, especially if I am over the age of 40, have been sedentary, or have any concerns about my cardiovascular or musculoskeletal health.
Assumption of Risk
I fully understand and acknowledge that:
Physical exercise involves inherent risks, including but not limited to muscle strains, sprains, fractures, joint injuries, cardiovascular events, and in rare cases, serious injury or death.
I am voluntarily participating in this event with full knowledge of the risks involved, and I freely and expressly assume all such risks.
I am responsible for listening to my body and stopping any activity that causes pain, dizziness, shortness of breath, chest discomfort, or any unusual symptom, and will immediately notify the instructor if this occurs.
I will inform the instructor of any limitations, injuries, or health concerns prior to beginning any exercise activity, and I take full responsibility for any failure to do so.
Release of Liability & Indemnification
In consideration of being permitted to participate in this event, I, for myself and on behalf of my heirs, assigns, and personal representatives, hereby:
RELEASE AND HOLD HARMLESS the event organizer, instructor(s), venue, sponsors, volunteers, and all associated individuals and entities (collectively, "Released Parties") from any and all claims, demands, losses, damages, costs, and liabilities arising out of or related to my participation in this event, including those caused by the negligence of the Released Parties.
AGREE TO INDEMNIFY AND HOLD HARMLESS the Released Parties from any loss, liability, damage, or costs they may incur due to my participation in this event.
Agreement
I have read this waiver in its entirety and fully understand its contents. I am agreeing freely and voluntarily, without any inducement or assurance, and intend this to be a complete and unconditional release of all liability to the greatest extent allowed by law.
